Physical therapy forms need to cover the related aspects of client-patient management, initial evaluation / management, re-examination, visits and discharge. Each visit done by the patient needs to be supported by physical therapy forms. The name of the patient and identification number needs to be documented in every form. All the physical therapy forms which are related to evaluation, examination, discharge and visits need to be authenticated by physical therapist providing the service. These forms need to clearly indicate the details like the patient being referred by a doctor or if he has come directly. If the therapy forms are covers ‘No fault insurance’ then, the patient needs to fill in the appointment policy form, information form, benefit assignment form, authorization form and history form. Patients who are not covered by the insurance covers need to fill up the form with the details like personal information, health questions, health survey, financial policy, history, authorization and appointment policy.
