Getting to the Bottom of an Issue with Root Cause Analysis

He sees that the puddle is caused by a leak in an overhead pipe due to high water pressure. The manager further investigates and realizes the water pressure valve is faulty. If the manager hadn’t investigated the root cause, the water problem surely will recur and cause more damage.
Root cause analysis is an accurate description of what and how things happened, preventing recurrence. Root causes are also identifiable and may be controlled by management with further recommendations to fix the situation. The process of RCA may range from simple to complex, but generally involves data collection, cause charting, root cause identification, and recommendation generation and implementation.
